DO NOT submit a statement form online without first speaking with an officer. If appropriate, an officer will either direct you to this page to complete a statement, or email you a link to complete the form.

The destination folder for online statements is not regularly monitored. You must first contact the Madison Township Police Dispatch Center at 440-428-2116 to speak with an officer and initiate your report.

 

If you submit a statement without first completing this step, it will not be recorded or responded to.
